Roles & Responsibilities

• Assess and evaluate speech, language, cognitive-communication, and swallowing function for SNF residents
• Develop individualized, evidence-based treatment plans and measurable goals
• Deliver skilled SLP therapy to support residents' safe swallowing and communication needs
• Collaborate with nursing, PT/OT, and interdisciplinary teams to coordinate care and maximize outcomes
• Document timely and accurate progress notes, plans of care, and discharge summaries in the facility EHR
• Educate residents, families, and staff on therapy plans, safe swallowing strategies, and communication supports
• Monitor and adjust interventions based on resident progress and team feedback

Qualifications

• ASHA Certificate of Clinical Competence (CCC) — listed as a primary requirement
• 1-3 years of SLP experience (experience in Skilled Nursing Facility settings preferred)
• Valid state SLP license or ability to obtain licensure in Tennessee (please verify state requirements)
• Strong clinical skills in dysphagia management, cognitive-communication, and rehabilitation
• Excellent documentation, communication, and interdisciplinary collaboration skills
• Current basic life support/CPR certification as required by facility policies (if applicable)

Knowledge & Skills

• Expertise in speech-language pathology evaluation and intervention for older adults
• Skilled in dysphagia assessment and safe-swallowing strategies
• Familiarity with SNF workflows, regulatory documentation, and therapy plan of care practices
• Proficient documentation skills in electronic health records
• Ability to collaborate effectively with interdisciplinary teams and family members
• Strong clinical reasoning and outcome-focused treatment planning
