Roles & Responsibilities

• Serve as charge nurse on shift — lead the team, coordinate care, and ensure smooth handoffs
• Administer medications safely and accurately, following facility protocols
• Perform wound care with attention to detail and best-practice techniques
• Document resident care in the Matrix EMR to keep charts up to date and compliant
• Collaborate with PT and social work designee to support interdisciplinary care plans
• Communicate with on-call physicians as needed to escalate concerns
• Maintain resident safety and comfort in a long-term care setting serving patients with heart disease, diabetes and dementia

Qualifications

• Active RN license (eligible to practice in the U.S.) — License to submit required
• Current BLS certification (required)
• Minimum ~2 years nursing experience (posting lists 2 years) with Long Term Care experience required
• Experience as a charge nurse or demonstrated leadership on shift
• Hands-on medication administration and wound care experience
• Familiarity with Matrix/MatrixCare or similar long-term care EMR
• Comfortable working night shifts and meeting weekend/holiday rotation requirements

Education

• Associate Degree in Nursing (ADN) or nursing diploma acceptable; BSN not required

Knowledge & Skills

• Medication administration best practices and MAR documentation
• Wound assessment and dressing application
• Long-term care resident needs including dementia, cardiac and diabetic care
• MatrixCare charting and electronic documentation workflows
• Working knowledge of concentrator and portable oxygen equipment
• Collaborative communication with PTs and social work liaisons
• Charge nurse skills: delegation, prioritization and team coordination
