Roles & Responsibilities

• Assess and evaluate patients in their homes to create personalized treatment plans
• Deliver evidence-based physical therapy interventions to improve mobility and function
• Document patient progress and treatment plans accurately in the medical record
• Collaborate with multidisciplinary team members and caregivers to coordinate care
• Educate patients and families on home exercise programs and fall prevention
• Complete Trinity specialty competency testing and skills checklists as required
• Ensure compliance with license verification and organizational policies

Qualifications

• Current, unrestricted Physical Therapist license (state licensure/verification required)
• Basic Life Support (BLS) certification
• Trinity-specific competency testing and completion of skills checklist required
• Immunizations and health screening up to date per Trinity requirements (MMR, Hepatitis B, COVID-19, Influenza, Tdap, Varicella, 2-step PPD or IGRA, drug screen, fit test, color vision, physical/health statement)
• Epic experience preferred but not required
• Comfortable providing care in patient homes and documenting thoroughly

Knowledge & Skills

• Experience in home health physical therapy practices and safety in the home environment
• Strong patient assessment and care planning skills
• Excellent communication and patient education abilities
• Proficient documentation skills; Epic experience is a plus
• Ability to work independently and prioritize caseloads
• Familiarity with mobility, balance, and functional training techniques
