Roles & Responsibilities

• Perform detailed patient assessments by monitoring vital signs and conducting targeted examinations to guide care plans
• Administer medications and injections accurately while following established protocols and safety checks
• Maintain thorough and timely medical documentation in the EMR with strict HIPAA compliance
• Collect and process specimens and assist with basic laboratory testing to support physician evaluations
• Provide patient education and health coaching on medication management, nutrition, and self-care
• Collaborate with physicians, RNs, and clinic staff to ensure smooth patient flow and high-quality care
• Monitor patients for changes in condition and escalate concerns promptly to the clinical team

Qualifications

• Licensed as an LPN in the State of Illinois (required)
• Graduate of an approved school of nursing (LPN program)
• BLS certification (required) and comfort with basic life support procedures
• Experience with EMR systems and strong medical documentation skills
• Experience in ambulatory care preferred but motivated new grads with solid fundamentals are welcome
• Comfort with extended computer input, telephone documentation, and sitting for long periods
• Willingness to follow infection control procedures; may be exposed to communicable diseases

Education

• Graduate of an approved school of nursing (LPN program) or equivalent diploma/certificate
