Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents as an LPN
• Administer medications and treatments safely and document accurately
• Monitor resident conditions, report changes, and escalate concerns promptly
• Collaborate with RNs, physicians, and the care team to support individualized care plans
• Assist residents with activities of daily living and promote comfort and dignity
• Maintain accurate charts, follow facility policies, and uphold infection control standards

Qualifications

• Active Licensed Practical Nurse (LPN) credential (required)
• Graduate of an accredited practical nursing program or equivalent training
• Current basic clinical skills in medication administration and patient assessment
• Ability to work night shifts and adapt to per-diem scheduling
• Strong communication and teamwork skills; professionalism and reliability

Education

• Practical Nursing diploma or certificate from an accredited program

Knowledge & Skills

• Medication administration and safe documentation practices
• Clinical assessment of vital signs and basic patient condition monitoring
• Charting and electronic health record familiarity
• Infection control best practices and resident safety focus
• Clear communication with residents, families, and interdisciplinary teams
