Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in a rehab setting
• Administer medications and treatments accurately and document per policy
• Monitor patients’ conditions, report changes to the care team, and escalate when needed
• Assist with rehabilitation activities and mobility programs to support recovery
• Collaborate with nurses, therapists, and physicians to deliver coordinated care
• Maintain accurate clinical records in the electronic medical record and complete required paperwork
• Promote a safe environment by following infection control and safety protocols

Qualifications

• Active Licensed Practical Nurse (LPN) credential — required
• Current Basic Life Support (BLS) or CPR certification preferred/required
• Previous experience in long-term care, rehab, or acute care is a plus
• Strong communication skills and ability to work well on an interdisciplinary team
• Ability to pass facility background checks and meet health screening requirements

Education

• Completion of an accredited Practical Nursing program (diploma or certificate)
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation accuracy
• Vital signs monitoring and basic patient assessment
• Familiarity with rehabilitation care and restorative nursing principles
• Proficiency with electronic medical records (EMR) or willingness to learn
• Patient safety, infection control, and fall prevention practices
• Compassionate patient interaction and family education
