Location: Albuquerque, NM

Contract Type: Per-Diem

Shift Type: Per diem availability, Days vary (per diem), 6:00 AM - 6:30 PM (Day shift)

Hours per Week: 36

Candidate Pay Rate: $31 per hour

Roles & Responsibilities

Provide compassionate direct patient care to residents at the rehab center
Administer medications and treatments safely and document care accurately
Monitor, record and report changes in patient condition to the care team
Assist with activities of daily living to promote patient comfort and dignity
Collaborate with RNs, therapists and other staff to implement care plans
Perform routine assessments, vital signs, and timely interventions
Maintain accurate electronic and paper charting in accordance with facility standards
Follow infection prevention and safety protocols to protect patients and staff

Qualifications

Current LPN license in New Mexico (required)
Basic Life Support (BLS/CPR) certification (required or obtain upon hire)
Prior experience in rehab, long-term care, or similar settings preferred
Ability to pass background check and required health screenings
Strong communication skills and a team-oriented attitude
Flexible availability to accept per diem shifts

Education

Graduate of an accredited Practical Nursing program
Current LPN licensure

Knowledge & Skills

Medication administration and documentation best practices
Patient assessment and basic clinical monitoring
Familiarity with electronic health records and charting
Infection control and standard precautions
Time management and prioritization in a fast-paced environment
Empathy, patient education and clear communication with families
