Roles & Responsibilities
- Provide compassionate direct patient care to residents at the rehab center
- Administer medications and treatments safely and document care accurately
- Monitor, record and report changes in patient condition to the care team
- Assist with activities of daily living to promote patient comfort and dignity
- Collaborate with RNs, therapists and other staff to implement care plans
- Perform routine assessments, vital signs, and timely interventions
- Maintain accurate electronic and paper charting in accordance with facility standards
- Follow infection prevention and safety protocols to protect patients and staff
Qualifications
- Current LPN license in New Mexico (required)
- Basic Life Support (BLS/CPR) certification (required or obtain upon hire)
- Prior experience in rehab, long-term care, or similar settings preferred
- Ability to pass background check and required health screenings
- Strong communication skills and a team-oriented attitude
- Flexible availability to accept per diem shifts
Education
- Graduate of an accredited Practical Nursing program
- Current LPN licensure
Knowledge & Skills
- Medication administration and documentation best practices
- Patient assessment and basic clinical monitoring
- Familiarity with electronic health records and charting
- Infection control and standard precautions
- Time management and prioritization in a fast-paced environment
- Empathy, patient education and clear communication with families
