Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to adults in the facility
• Administer medications and treatments safely and document them accurately
• Perform focused patient assessments and report clinical changes promptly
• Collaborate with RNs, physicians, and the care team to support care plans
• Assist patients with activities of daily living while promoting dignity and independence
• Maintain accurate and timely electronic charting and handoffs
• Adhere to infection control standards and safety protocols at all times
• Support patient and family education with clear, empathetic communication

Qualifications

• Active Licensed Practical Nurse (LPN) license in New Hampshire (required)
• Completion of an accredited LPN program
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Ability to pass facility background checks and required health screenings
• Strong communication skills and a team-oriented mindset
• Per diem availability and flexibility for variable day shifts

Education

• Graduate of an accredited Practical Nursing (LPN) program

Knowledge & Skills

• Medication administration and basic clinical procedures
• Patient assessment and vital signs monitoring
• Wound care basics and infection prevention practices
• Proficient documentation in electronic health records
• Time management and prioritization in a fast-paced setting
• Clear, compassionate patient and family communication
• Familiarity with long-term care/center workflows (helpful)
• Ability to work collaboratively with interdisciplinary teams
