Physical Therapist - Home Care

Location: Wall Township, NJ

Contract Type: Contract

Shift Type: Full-time, typically 40 hours/week; daytime visits with flexible start/end times based on patient caseload (Monday-Friday typical; exact times vary)

Hours per Week: 40

Candidate Pay Rate: $60- $70 per hour

Roles & Responsibilities

Perform comprehensive patient evaluations in the home setting to assess mobility, strength, balance, and functional needs
Develop and implement individualized treatment plans that promote independence and improve quality of life
Provide therapeutic exercises, gait training, balance training, and manual therapy as clinically appropriate
Educate patients and caregivers on home exercise programs and safety techniques to support long-term outcomes
Monitor and document patient progress in compliance with agency and regulatory standards
Communicate effectively with physicians, nurses, and case managers to coordinate care
Participate in care plan meetings and contribute clinical expertise to interdisciplinary teams

Qualifications

Active New Jersey Physical Therapist license required
Graduate of an accredited Physical Therapy program (DPT or equivalent)
Current CPR/BLS certification
Minimum 1 year of clinical experience; home health experience preferred
Reliable transportation and valid driver’s license for patient visits
Strong time management and organizational skills

Education

Doctor of Physical Therapy (DPT) or equivalent — graduate of an accredited Physical Therapy program

Knowledge & Skills

Physical Therapy evaluation and treatment planning
Therapeutic exercises and gait training
Balance training and manual therapy techniques
Developing and teaching home exercise programs
Patient education and safety techniques
Effective communication with interdisciplinary teams
Caseload management in a home health setting
