Roles & Responsibilities
- Evaluate patients to determine functional abilities and occupational needs
- Develop individualized, meaningful treatment plans that promote independence
- Implement therapeutic interventions to improve ADLs, strength, and coordination
- Collaborate with nurses, PTs, SLPs and the interdisciplinary team to drive patient-centered care
- Document assessments, progress notes and discharge plans accurately and on time
- Educate patients and caregivers on home programs, adaptive equipment and safety strategies
- Contribute to quality improvement initiatives and maintain regulatory compliance
Qualifications
- Licensed Occupational Therapist in New Hampshire (or COTA credential if applying as a Certified Occupational Therapy Assistant)
- Current BLS/CPR certification (or willingness to obtain prior to start)
- Ability to work the scheduled day shift (8:00 AM - 4:30 PM) and start on the assignment start date
- Strong clinical reasoning, documentation skills and patient-centered approach
- Reliable, professional, and ready to collaborate within an interdisciplinary rehab team
Knowledge & Skills
- Skilled in OT assessment and intervention for functional independence
- Experience with ADL training, adaptive equipment and caregiver education
- Proficient with timely clinical documentation and care planning
- Strong verbal and written communication with patients and team members
- Good time management, prioritization, and safe patient handling skills
