Shift Type: Monday-Friday, 8:00 AM - 4:30 PM Day (5x8-hour shifts; facility cannot accommodate 4x10s or 3x12s; no weekends required)

Candidate Pay Rate: $67 per hour

Roles & Responsibilities
- Assess patients in their homes to determine functional status and OT needs, with a focus on geriatrics and adult populations
- Develop and implement individualized treatment plans that address ADLs, mobility, safety, and caregiver education
- Document thoroughly using OASIS and EPIC Remote (laptop) plus Rover mobile charting to ensure continuity of care
- Provide hands-on modalities and manual therapy as appropriate (TENS, ultrasound, edema management, etc.)
- Perform fall and risk assessments and implement prevention strategies to keep patients safe at home
- Coordinate care with interdisciplinary team members and community resources to support patient outcomes
- Manage a focused caseload (may visit up to ~6 patients/day; typical productivity expectations apply)
- Maintain infection prevention practices and comply with facility guidelines
Qualifications
- Active Occupational Therapist license in Oregon or Washington (pending licensure accepted)
- BCLS / BLS certification required
- Minimum 1 year of skilled home health experience required
- Comfortable with OASIS documentation and EPIC/Rover charting
- Reliable personal transportation and willingness to travel (occasional daily radius up to ~60 miles)
- Ability to manage up to six home visits per day and maintain expected productivity
Knowledge & Skills
- OASIS documentation and home health charting workflows
- EPIC Remote client and Rover mobile charting
- Assessment and treatment of Activities of Daily Living (ADLs)
- Modalities and manual therapy skills (TENS, ultrasound, edema massage, electrical stimulation)
- Geriatric care, orthopedics, post-op rehab, and neuro diagnoses (stroke, Parkinson’s, ALS, MS)
- Fall and risk assessment, infection prevention practices
- Familiarity with National Patient Safety Goals and core measures (preferred)
- Experience with prosthetics/orthotics and splinting techniques (preferred)
