Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate, high-quality direct patient care to long-term and rehab residents
- Administer medications and treatments accurately and document thoroughly
- Perform timely patient assessments and report changes in condition to the care team
- Collaborate with nurses, therapists, and physicians to coordinate individualized care plans
- Support admissions, discharges, and transitions with clear communication and professionalism
- Maintain a safe, clean environment and follow infection control and facility policies
Qualifications
- Active LPN or RN license in Massachusetts (required)
- Current BLS (Basic Life Support) certification or equivalent
- Comfortable working in a long-term care / rehabilitation setting
- Strong clinical judgment, communication skills, and teamwork mindset
- Previous LTC/rehab experience preferred but per-diem friendly to experienced and returning clinicians
Education
- Completion of an accredited LPN program for Licensed Practical Nurses
- Completion of an accredited nursing program (ADN/BSN) for Registered Nurses
Knowledge & Skills
- Skilled in medication administration and basic nursing procedures
- Patient assessment and monitoring of vital signs and symptoms
- Accurate documentation and charting practices
- Ability to collaborate with interdisciplinary teams and communicate with families
- Time management and prioritization in a busy care environment
