Roles & Responsibilities
- Provide skilled physical therapy evaluations and treatments to home health and hospice patients
- Develop and implement individualized, goal-driven treatment plans
- Perform thorough patient assessments and document progress clearly in HomeCare/HomeBase
- Collaborate with interdisciplinary team members, including PTAs and rehab aides, to coordinate care
- Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
- Manage a caseload of approximately 5-7 patients per day while prioritizing quality and safety
- Drive safely to patient homes within the assigned coverage radius and maintain required driving documentation
Qualifications
- Active ND Physical Therapy License (required)
- Current BLS certification (required)
- Minimum 1 year of physical therapy experience, with home health experience preferred
- Valid Driver's License and current auto insurance (required)
- Comfortable documenting in HomeCare/HomeBase EMR
- Ability to work Monday-Friday daytime schedule (8:00 AM - 5:00 PM)
Education

- Clinical expertise in physical therapy for post-surgical, fall-risk, and homebound patients
- Skilled in home health and hospice care considerations
