Roles & Responsibilities

• Provide skilled physical therapy evaluations and treatments to home health and hospice patients
• Develop and implement individualized, goal-driven treatment plans
• Perform thorough patient assessments and document progress clearly in HomeCare/HomeBase
• Collaborate with interdisciplinary team members, including PTAs and rehab aides, to coordinate care
• Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs
• Manage a caseload of approximately 5-7 patients per day while prioritizing quality and safety
• Drive safely to patient homes within the assigned coverage radius and maintain required driving documentation

Qualifications

• Active ND Physical Therapy License (required)
• Current BLS certification (required)
• Minimum 1 year of physical therapy experience, with home health experience preferred
• Valid Driver's License and current auto insurance (required)
• Comfortable documenting in HomeCare/HomeBase EMR
• Ability to work Monday-Friday daytime schedule (8:00 AM - 5:00 PM)

Education
