Roles & Responsibilities
- Provide compassionate social work services to patients and families
- Conduct thorough psychosocial assessments and develop care plans
- Coordinate care across multidisciplinary teams to ensure seamless support
- Facilitate 5150 evaluations and involuntary hold processes when needed
- Connect patients with vital community resources and support services
- Advocate for patients' emotional and social wellbeing throughout their healthcare journey
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Strong assessment and crisis intervention skills
Knowledge & Skills
- Expertise in involuntary psychiatric evaluation processes
- Knowledge of California mental health regulations
- Strong communication and interpersonal abilities
- Experience with care coordination and discharge planning
- Understanding of community resources and support systems
