Roles & Responsibilities
- Provide compassionate care coordination and support to patients in need
- Conduct thorough assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Facilitate 5150 evaluations and support involuntary psychiatric holds when appropriate
- Advocate for patients and connect them with vital community resources
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Strong communication and interpersonal skills
Knowledge & Skills
- Expertise in crisis intervention and psychiatric evaluation
- Knowledge of California mental health laws and 5150 processes
- Ability to work collaboratively within multidisciplinary teams
- Strong documentation and care coordination abilities
