Location: Louisville, Kentucky

Start Date: 06-22-2026

End Date: 09-21-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services in home health settings
Conduct start of care assessments, recertifications, and evaluations
Develop personalized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
Deliver daily visits to help patients regain mobility and independence
Collaborate with clinical managers and educators for optimal patient outcomes

Qualifications

Current Physical Therapist license in Kentucky
BLS certification required
Home health experience preferred but not required
Strong communication and time management skills
Valid driver's license and reliable transportation

Knowledge & Skills

Home health physical therapy practices
Patient assessment and care planning
Common diagnoses including CHF, orthopedic conditions, and pneumonia
Electronic documentation and tablet-based charting
Independent decision-making in field settings
