Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct OASIS assessments and Start of Care evaluations for new patients
Help elderly patients recover from falls and fractures with personalized treatment plans
Manage a manageable caseload of 4-6 patient visits daily using our point system
Document visits and track time efficiently using provided tablets

Qualifications

Current Physical Therapy license in Indiana
Experience with OASIS assessments and Start of Care documentation
Comfortable working with elderly patients in rural settings
Reliable transportation for multi-county coverage

Knowledge & Skills

Physical Therapy expertise
OASIS assessment proficiency
Start of Care documentation skills
Rural healthcare experience preferred
Tablet and electronic documentation comfort
