Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate physical therapy care to patients across Sacramento County in their homes
Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
Help patients regain strength, mobility, and independence with personalized treatment plans
Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
Document visits thoroughly and maintain accurate mileage records for reimbursement

Qualifications

Active Physical Therapy Assistant license in California
Minimum six months of clinical experience preferred
Strong communication and time-management skills
Comfortable working independently in home health settings
Valid driver's license and reliable transportation

Knowledge & Skills

Home health documentation and productivity standards
Treating a variety of diagnoses in community settings
Building rapport with patients and families
Navigating mileage reimbursement processes
Meeting productivity goals of 5-6 patients per day
