Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Candidate Pay Rate: $49 per hour

Roles & Responsibilities
- Deliver compassionate care to patients across cardio, pulmonary, orthopedic, neurological, and geriatric populations in their homes
- Evaluate and treat 5-6 patients daily while meeting productivity goals
- Collaborate with interdisciplinary teams to create personalized treatment plans
- Document patient progress and communicate updates with home health supervisors
- Travel throughout greater Sacramento county to provide essential therapy services
Qualifications
- Active Physical Therapy Assistant license in California
- Minimum experience preferred, new grads considered in exceptional cases
- Strong clinical skills across multiple patient populations
- Reliable transportation and willingness to travel locally
Knowledge & Skills
- Home health experience preferred
- Excellent time management and organizational abilities
- Strong communication and interpersonal skills
- Ability to work independently in various home settings
- Proficiency with documentation and electronic medical records
