Location: Louisville, Kentucky

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy services in home health settings
- Conduct start of care assessments and evaluations for new patients
- Perform recertifications and resume of care visits as needed
- Deliver daily therapy visits to patients with CHF, orthopedic conditions, and pneumonia
- Collaborate with clinical managers and educators for optimal patient outcomes
Qualifications
- Current Physical Therapist license in Kentucky
- BLS certification required
- Home health experience preferred but not required
- Strong communication and documentation skills
- Ability to work independently in the field
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and evaluation skills
- Knowledge of CHF, orthopedic, and pneumonia rehabilitation protocols
- Electronic documentation proficiency
- Time management and organizational abilities
