Location: Dickinson, North Dakota

Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy care to patients in their homes
- Conduct start of care assessments, recertifications, and resumption of care visits
- Deliver 6.5 points of patient care per day with a mix of regular visits and admissions
- Collaborate with clinical coordinators and directors for support and guidance
- Complete safety evaluations and teaching visits in hospice situations when needed
- Document patient progress and maintain accurate clinical records
Qualifications
- Current ND Physical Therapy License required
- BLS certification required
- Valid driver's license and auto insurance required
- Home Health experience preferred
- OASIS training preferred
- First time travelers welcome to apply
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Strong assessment and clinical decision-making skills
- Excellent communication and patient education abilities
- Ability to work independently while staying connected with the care team
- Comfortable with OASIS documentation and start of care processes
