Candidate Pay Rate: $70 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes throughout Tehama County
- Conduct Start of Care visits especially for orthopedic patients when nursing is not ordered
- Develop personalized treatment plans for orthopedic, joint replacement, and geriatric patients with multiple comorbidities
- Complete approximately 5 meaningful patient visits per day using the HCHB EMR system
- Collaborate with the intake RN and manager to coordinate comprehensive care
- Travel within the service area while enjoying mileage reimbursement support
Qualifications
- Current Physical Therapy license in California
- BLS certification required
- Home Health experience preferred but will consider candidates with less than 2 years experience who have Home Health background
- Strong communication and documentation skills
Knowledge & Skills
- Physical Therapy expertise in home health settings
- Experience with orthopedic and geriatric patient populations
- Proficiency with HomeCare HomeBase EMR system
- Ability to work independently while providing excellent patient care
