Roles & Responsibilities

Provide compassionate physical therapy services directly in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs while developing personalized care plans
Guide patients and families through exercises and the proper use of therapeutic equipment
Monitor progress and communicate changes to physicians to ensure optimal outcomes
Collaborate with home health aides and caregivers to support each patient's rehabilitation goals
Document clinical progress and prepare reports that help drive continued care decisions

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Home health physical therapy expertise
Ability to evaluate home environments for safety and rehab potential
Experience training patients on prosthetic devices when needed
Understanding of Medicare, Medi-Cal, and Title 22 regulations
Comfortable working independently in various home settings
