Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Deliver personalized physical therapy services to patients recovering from surgery, falls, and other conditions in their homes
Develop individualized treatment plans that help patients regain strength, mobility, and independence
Collaborate with an interdisciplinary care team to ensure seamless, patient-centered care
Educate patients and families on exercises and safety techniques to support recovery
Document progress accurately using the HomeCare/HomeBase EMR system
Travel within a 60-mile radius to provide care to home health and hospice patients

Qualifications

Current North Dakota Physical Therapy License
BLS certification required
At least 1 year of physical therapy experience, including previous home health experience
Valid driver's license and auto insurance
Strong communication and time-management skills

Knowledge & Skills

Home health physical therapy expertise
Ability to manage a caseload of 5-7 patients per day
Comfortable working independently in patients' homes
Familiarity with HomeCare/HomeBase EMR
Compassionate approach to caring for home health and hospice patients
