Roles & Responsibilities

• Perform comprehensive patient evaluations to assess mobility, strength, and functional needs
• Design and implement individualized treatment plans that promote independence and recovery
• Deliver targeted physical therapy interventions and therapeutic modalities to improve outcomes
• Educate patients and families on exercises, safe movement techniques, and home modifications
• Collaborate closely with occupational therapists, nurses, physicians, and the care team to coordinate holistic care
• Monitor patient progress and adjust plans to ensure the best possible functional gains
• Maintain accurate, timely documentation of assessments, progress notes, and treatment plans in accordance with facility policies
• Ensure treatment plans meet regulatory standards and best practices in physical therapy

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of California
• Experience working in a skilled nursing facility or similar rehabilitation setting preferred
• Strong knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment
• Excellent communication, interpersonal, and problem-solving abilities
• Ability to work independently while contributing positively to a collaborative team
• Compassionate, patient-centered approach to care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (accredited program required)
