Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to help them regain independence
• Develop and implement individualized treatment plans focused on maximizing functional outcomes
• Collaborate with an interdisciplinary team to coordinate care and optimize resident progress
• Document assessments, treatment plans, and progress notes accurately and in a timely manner
• Participate in care conferences and quality improvement efforts to enhance resident care

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical assessment and treatment planning skills
• Excellent communication and interdisciplinary collaboration abilities
• Skilled nursing or post-acute experience preferred
• Dependable, flexible, and team-oriented approach to patient care
