Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across assigned geographical regions
Evaluate and treat patients with orthopedic conditions, strokes, joint replacements, and spinal injuries
Complete OASIS documentation to meet government-mandated requirements
Collaborate with interdisciplinary teams including OT and Speech Therapy
Develop individualized care plans that promote recovery and independence
Provide education and support to patients and families throughout their rehabilitation journey

Qualifications

Active Physical Therapist license in California
Minimum 1 year of experience working in PT, OT, or Speech Therapy
Experience with OASIS documentation required
BLS certification required
Home health setting experience preferred but not required
Strong communication and organizational skills

Knowledge & Skills

Proficiency with OASIS documentation and compliance standards
Cerner EMR system experience
Ability to work independently in home-based care settings
Knowledge of orthopedic, neurological, and post-surgical rehabilitation
Excellent time management and patient assessment skills
Comfortable with business attire and white lab coat uniform requirements
