Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Perform start of care, recertifications, and resumption of care visits for home health patients
- Complete an average of 6.5 points per day, including daily visits and occasional admissions
- Collaborate with the interdisciplinary team to develop and adjust patient care plans
- Provide skilled physical therapy interventions to help patients regain independence
- Document patient progress accurately and timely in accordance with agency standards
Qualifications
- Current North Dakota Physical Therapy license
- BLS certification from the American Heart Association
- Valid driver's license and auto insurance
- Home health experience and OASIS training preferred
- Comfortable with a 50-mile radius rule and not currently employed by CommonSpirit/CHI/Dignity
Knowledge & Skills
- Strong knowledge of home health regulations and OASIS documentation
- Excellent time management and organizational skills
- Ability to work independently and make clinical decisions in the field
- Compassionate communication with patients and families
