Roles & Responsibilities

• Provide physical therapy services within the legal scope of practice, following all licensure requirements
• Conduct initial evaluations, progress notes, 30-day reassessments, and 60-day recertifications in accordance with Medicare and state standards
• Develop, implement, and update individualized plans of care aligned with physician orders and patient needs
• Deliver skilled interventions and maintenance therapy programs as permitted by state scope of practice
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides in compliance with state practice acts
• Educate patients and caregivers on home exercise programs, mobility, and adaptive strategies to promote independence and safety
• Prepare and submit OASIS assessments, recertifications, transfers, and discharges within agency timeframes
• Apply ICD-10 coding for accurate and compliant documentation

Qualifications

• Graduate of an accredited physical therapist curriculum approved by CAPTE, APTA, or CAHEA
• Currently licensed as a Physical Therapist in the state of practice
• Two years of clinical experience preferred; home health experience a plus
• Current CPR certification
• Reliable transportation, valid driver's license, and auto insurance for home visits

Education

• Graduate of an accredited physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation

Knowledge & Skills

• Physical Therapy evaluation and treatment
• OASIS documentation and ICD-10 coding
• Care plan development and patient education
• Supervision of PTAs and Home Health Aides
• EMR systems and basic computer proficiency
• Strong communication and interpersonal skills
• Commitment to compliance and quality improvement
