Roles & Responsibilities

• Implement individualized treatment plans designed by the physical therapist to help residents regain mobility and independence
• Assist with therapeutic exercises, gait training, transfers and ADL support while monitoring patient response
• Document treatment progress accurately and timely in the facility’s records to support continuity of care
• Collaborate with nurses, CNAs and rehab team members to coordinate care and ensure safety
• Educate residents and families on home programs, safety techniques and adaptive strategies
• Maintain treatment areas and equipment, adhering to infection control and safety standards

Qualifications

• Current PTA licensure required (client will not wait for pending licensure)
• Skilled Nursing Facility (SNF) experience required — this role is not open to new grads
• Ability to work within a 25-mile float radius; reliable transportation and valid driver’s license expected
• Comfortable working the guaranteed 35-hour work week and adaptable to facility scheduling needs
• Strong communication skills and ability to collaborate in an interdisciplinary team

Knowledge & Skills

• Hands-on experience with therapeutic exercise, transfer training and gait assistance
• Proficient documentation practices for SNF settings and familiarity with weekly productivity/cycle expectations
• Patient-centered approach with strong safety and fall-prevention techniques
• Ability to prioritize cases, manage time across a caseload and communicate clearly with staff and families
• Basic equipment handling and maintenance knowledge (e.g., walkers, Hoyer lifts, assistive devices)
