Perm Occupational Therapist

Location: Fremont, CA

Contract Type: Perm

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day shift, 35 hours/week, No weekend rotation)

Hours per Week: 35

Candidate Pay Rate: $54 per hour

Roles & Responsibilities

Evaluate patients' functional abilities and identify occupational therapy needs
Develop individualized treatment plans that promote independence and meaningful participation
Provide skilled therapeutic interventions using evidence-based techniques
Collaborate with nurses, physicians, and the interdisciplinary team to coordinate care
Document progress and outcomes clearly and timely in the patient record
Educate patients and families on strategies and adaptive techniques to support recovery
Measure and report functional outcomes to support continuous improvement

Qualifications

Licensed Occupational Therapist or eligible for licensure in California
Clinical experience in post-acute, rehabilitation, or similar settings preferred
Strong clinical reasoning with the ability to develop and implement treatment plans
Excellent communication skills for patient education and team collaboration
Current CPR/BLS certification preferred

Education

Degree in Occupational Therapy (OT) from an accredited program

Knowledge & Skills

Skilled in occupational therapy evaluation and standardized assessments
Knowledge of therapeutic modalities, adaptive equipment, and functional task training
Comfortable with documentation in electronic medical records
Ability to work effectively within an interdisciplinary team
Patient- and family-centered education and discharge planning
