Roles & Responsibilities

• Provide high-quality nursing care to long-term care residents with compassion and professionalism
• Administer medications and treatments accurately according to care plans
• Perform thorough assessments and document resident status clearly and timely
• Collaborate with interdisciplinary team members to coordinate resident care
• Support resident safety, mobility, and activities of daily living through hands-on care and education
• Communicate effectively with families and care teams to promote excellent outcomes
• Participate in shift handoffs and maintain accurate, compliant clinical records

Qualifications

• Current LPN license in New York or compact-state equivalent where applicable
• Experience in long-term care preferred; comfortable with 12-hour day shifts
• Active Basic Life Support (BLS) certification or willingness to obtain
• Valid immunizations and TB screening as required by the facility
• Strong communication and teamwork skills with a resident-centered approach
• Ability to complete credentialing quickly for a rapid start

Education

• Licensed Practical Nurse (LPN) diploma or equivalent training

Knowledge & Skills

• Medication administration and documentation best practices
• Wound care, ADL support, and chronic condition management
• Familiarity with long-term care workflows and electronic charting
• Patient assessment, infection control, and safety protocols
• Time management and priority-setting during busy 12-hour shifts
