Contract Type: Per-Diem

Shift Type: 8-hour shifts; specific start/end times and days to be confirmed

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on patient care and monitor vital signs
Administer medications and treatments safely and accurately
Document patient information clearly and timely in the chart
Collaborate with RNs and interdisciplinary team to support care plans
Assist with activities of daily living and patient comfort
Observe and report changes in patient condition to the care team
Support admissions, discharges, and smooth patient transitions

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Comfortable with medication administration and charting
Strong communication skills and ability to work in a team
Preferred: prior experience in long-term care, rehab, or acute care

Education

Practical Nursing diploma or vocational nursing certificate; high school diploma or GED

Knowledge & Skills

Vital signs assessment and basic clinical monitoring
Medication administration and safe documentation practices
Patient hygiene, mobility assistance, and ADL support
Basic wound care and infection control procedures
Familiarity with electronic health records and charting
