Roles & Responsibilities

• Provide compassionate, hands-on nursing care to patients consistent with LPN scope of practice
• Administer medications and treatments accurately, following facility protocols
• Monitor and record vital signs and patient status, escalating changes to the care team
• Collaborate with RNs, providers, and support staff to deliver coordinated care
• Maintain clear, timely documentation in the patient record and communicate handoffs
• Support patient and family education, promoting comfort and understanding
• Adhere to infection control, safety protocols, and regulatory standards

Qualifications

• Active New York Licensed Practical Nurse (LPN) license in good standing
• Current BLS/CPR certification or willingness to obtain upon hire
• Completion of an accredited practical nursing program
• Ability to pass background check and meet facility immunization requirements
• Strong interpersonal skills and comfort working in a team-based setting
• Prior experience in long-term care or similar settings is a plus

Education

• Completion of an accredited LPN / Practical Nursing program

Knowledge & Skills

• Solid knowledge of medication administration and charting best practices
• Skilled at taking and interpreting vital signs and basic clinical assessments
• Familiarity with electronic medical records and timely documentation
• Good patient education and communication skills for diverse populations
• Understanding of infection prevention, safety, and standard precautions
• Ability to prioritize tasks and work efficiently in a fast-paced environment
