Roles & Responsibilities

Provide compassionate, direct patient care with attention to safety and dignity
Administer medications and treatments accurately and timely
Monitor and record vital signs, reporting changes promptly
Collaborate with nurses and interdisciplinary team members to coordinate care
Maintain clear, accurate documentation in the patient record
Assist with admissions, discharges and patient transfers as needed

Qualifications

Current LPN license (Licensed Practical Nurse) — Pennsylvania license recommended
Graduation from an accredited practical nursing program
Current BLS/CPR certification (or willingness to obtain)
Ability to pass required background checks and health screenings
Strong communication skills and a patient-centered mindset

Education

Diploma or certificate from an accredited practical nursing (LPN) program

Knowledge & Skills

Medication administration and basic clinical assessments
Wound care and basic restorative nursing skills
Electronic health record (EHR) charting and documentation
Effective communication with patients, families and clinical teams
Time management, organization and infection control principles
