Roles & Responsibilities

• Provide compassionate direct patient care within the scope of LPN practice, making each patient feel seen and supported
• Administer medications and treatments accurately and document all interventions clearly
• Monitor patients, recognize changes in condition, and promptly report concerns to the RN or provider
• Collaborate closely with RNs, nursing assistants, and interdisciplinary team members to coordinate excellent care
• Maintain accurate and timely patient records and charting in the facility’s documentation system
• Assist with admissions, discharges, and patient education to promote safe transitions and patient understanding

Qualifications

• Current LPN license in Pennsylvania (required)
• Graduation from an accredited practical nursing program
• Current BLS/CPR certification or willingness to obtain upon hire
• Up-to-date immunizations and ability to pass required background checks
• Strong clinical judgment, communication skills, and a patient-centered approach
• Previous LPN experience preferred but motivated new grads are encouraged to apply

Education

• Graduate of an accredited practical nursing program
• Active LPN license in Pennsylvania

Knowledge & Skills

• Clinical assessment and basic patient monitoring
• Medication administration and documentation best practices
• Wound care fundamentals and basic nursing procedures
• Electronic health record (EHR) competency for accurate charting
• Infection control and safety protocols
• Clear communication, teamwork, and time management in a fast-paced setting
