Location: Andover, MA

Contract Type: Per-Diem

Shift Type: Per diem / Variable days, 3:00 PM - 11:00 PM (day shift)

Hours per Week: 40

Candidate Pay Rate: $47 per hour

Roles & Responsibilities

Provide safe, compassionate direct patient care including medication administration and treatments
Perform timely assessments and document changes in patient condition clearly and accurately
Coordinate with the interdisciplinary team to develop and follow individualized care plans
Assist with admissions, transfers and discharges to ensure smooth transitions
Support rehabilitation activities and help residents toward their recovery goals
Maintain infection control standards and a safe environment for residents and staff
Communicate effectively with residents, families and team members to promote excellent care

Qualifications

Active LPN license in Massachusetts (required)
Current BLS/CPR certification (required or obtain upon hire)
Prior experience in long-term care, rehabilitation, or skilled nursing preferred
Up-to-date immunizations and ability to pass background/credential checks
Strong interpersonal and communication skills; compassionate, resident-focused mindset

Education

Graduate of an accredited LPN/Vocational Nursing program

Knowledge & Skills

Medication administration and documentation accuracy
Basic wound care and restorative nursing skills
Skilled in resident assessment and care planning
Familiarity with electronic health records and charting best practices
Ability to work well in a fast-paced team environment and prioritize tasks
Good time management and critical thinking under pressure
