Roles & Responsibilities

• Coordinate patient care across the Surgery Outpatient Clinic by proactively managing case workflows
• Collaborate with nursing staff and providers to ensure seamless transitions and timely follow-up
• Conduct patient assessments and develop individualized care plans emphasizing safety and outcomes
• Serve as the point person for care coordination, communicating clearly with patients, families, and the care team
• Maintain accurate, timely documentation in the medical record and track case progress
• Facilitate referrals, community resources, and post-op planning to support recovery and reduce readmissions
• Participate in team meetings and contribute to process improvements that enhance patient care

Qualifications

• Active Registered Nurse (RN) credential (required)
• Minimum three (3) years of case management experience (required)
• Experience working in clinic-based or outpatient surgical settings (preferred)
• Strong collaboration and communication skills with interdisciplinary teams
• Comfortable with clinical documentation and care coordination workflows

Education

• Active Registered Nurse (RN) license
