Roles & Responsibilities

• Provide respectful, skilled assistance with activities of daily living (bathing, dressing, toileting, feeding) for residents in a skilled nursing setting
• Monitor and record vital signs and resident status accurately and promptly
• Assist the nursing team with safe transfers, mobility support, and fall-prevention measures
• Communicate clearly and compassionately with residents and families to support person-centered care
• Document resident care in facility records per policy and help ensure timely care planning
• Follow infection prevention protocols and state vaccine mandates to keep residents and staff safe
• Collaborate with interdisciplinary team members to support quality outcomes and resident dignity

Qualifications

• Current, unrestricted Colorado CNA certification (required)
• Experience in Long Term Care / Skilled Nursing Facility (LTC experience required)
• COVID-19 vaccination as required by state mandate
• Compassionate, dependable, and team-oriented with good communication skills
• Ability to perform physically safe resident care and follow facility policies

Education

• Current, unrestricted Colorado CNA certification

Knowledge & Skills

• Skilled in ADLs and basic resident care in a long-term care environment
• Comfortable taking and documenting vital signs and observing changes in condition
• Knowledge of infection control practices and state vaccination requirements
• Strong interpersonal skills for working with residents, families, and the care team
• Ability to follow care plans, facility protocols, and accurate charting practices
