Roles & Responsibilities

• Assist with coordinating virtual physicals and patient communication to ensure smooth telehealth encounters
• Provide direct patient care including assistance with activities of daily living and monitoring basic health indicators
• Document observations and care provided accurately and promptly in patient records
• Collaborate with the nursing team and allied health staff to support continuity of care
• Maintain a safe, respectful environment for patients and adhere to facility policies and procedures

Qualifications

• Current, valid Certified Nursing Assistant (CNA) certification
• Comfort and experience working 8:00 AM - 4:00 PM, Monday through Friday
• Familiarity or willingness to assist with virtual physical coordination and telehealth workflows
• Strong communication skills and the ability to document care clearly
• Ability to pass any required facility screenings and comply with site-specific requirements

Education

• High school diploma or GED preferred
• Current CNA certification required

Knowledge & Skills

• Knowledge of basic nursing and patient care techniques expected of a CNA
• Skill in patient observation, reporting changes, and following care plans
• Comfort with technology used for virtual coordination of physicals
• Ability to work collaboratively within a multidisciplinary team
• Attention to detail and adherence to infection control and safety protocols
