Roles & Responsibilities

• Manage routine case management, discharge planning, and utilization review for a diverse patient population
• Coordinate swing bed placements and complete MDS documentation as needed
• Screen potential admissions for the swing bed program and ensure Medicare, Medicaid, HMO, and private insurance criteria are met
• Collaborate with physicians, bedside staff, and ancillary teams during daily rounds to facilitate timely discharges
• Communicate with patients and families to assess discharge needs and provide ongoing education
• Obtain referrals and prior authorizations for home health, CME, SNF, acute rehab, and follow-up appointments
• Educate physicians and staff on appropriate levels of care and documentation requirements
• Run Interqual reviews for each inpatient and observation patient admitted

Qualifications

• Active RN license in the state of New Mexico (must be currently active)
• Minimum 5 years of experience in case management, utilization review, or discharge planning
• Experience with swing bed coordination and MDS documentation
• Proficiency in Interqual criteria and insurance verification processes
• Strong communication and collaboration skills to work with multidisciplinary teams

Knowledge & Skills

• Expert knowledge of case management and discharge planning processes
• Working knowledge of Medicare, Medicaid, HMO, and private insurance coverage criteria
• Skilled in utilization review and utilization management
• Proficient in Interqual and MDS software
• Ability to coordinate swing bed placements and manage patient transitions
• Excellent documentation and organizational skills
