Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Conduct psychosocial assessments and develop care plans
- Coordinate care across multidisciplinary teams
- Provide crisis intervention and 5150 evaluations
- Advocate for patient needs and discharge planning
- Collaborate with community resources for continuity of care
Qualifications
- Valid California LCSW license
- BLS certification
- CA 5150 certification
- Prior experience at a Dignity Health facility required
Knowledge & Skills
- Strong clinical assessment and diagnostic skills
- Knowledge of community resources and discharge planning
- Excellent communication and collaboration abilities
- Crisis intervention and de-escalation techniques
