Roles & Responsibilities

Coordinate patient care plans to ensure seamless transitions and optimal outcomes
Advocate fiercely for patients navigating complex healthcare systems
Collaborate with multidisciplinary teams to deliver compassionate, high-quality case management
Assess needs and connect patients to vital community resources
Monitor progress and adjust strategies to promote recovery and independence

Qualifications

Active RN license in good standing
Minimum 2 years of nursing experience, ideally in case management
BLS certification
Strong passion for patient advocacy and care coordination

Education

Associate or Bachelor's degree in Nursing (ADN/BSN)

Knowledge & Skills

Expertise in case management principles and practices
Excellent communication and interpersonal skills
Proficiency in electronic health records and documentation
Ability to thrive in fast-paced hospital environments
