Roles & Responsibilities

• Deliver compassionate, high-quality nursing care to post-acute patients
• Perform assessments, develop care plans, and coordinate with interdisciplinary teams
• Administer medications and treatments safely and accurately
• Monitor patient status and intervene promptly for changes in condition
• Document care clearly and timely to support continuity and compliance
• Educate patients and families to support recovery and successful transitions
• Support unit goals and mentor other staff through strong clinical leadership

Qualifications

• Active RN license in Pennsylvania or compact RN license
• Current BLS required; ACLS and other specialty certifications preferred
• Minimum 1 year of recent clinical nursing experience in post-acute, rehab, med-surg, or similar settings
• Ability to pass background checks and meet facility onboarding requirements
• Excellent communication skills and a team-first attitude

Education

• Associate Degree in Nursing (ADN) required; BSN preferred

Knowledge & Skills

• Strong assessment and critical thinking skills
• Skilled in medication administration and nursing documentation
• Familiarity with post-acute care workflows and discharge planning
• Comfortable working independently on night shift and managing urgent needs
• Proficient with electronic health records and basic computer tasks
• Customer-service mindset when interacting with patients and families
