Location: Carmichael, California

Start Date: 07-07-2026

End Date: 10-06-2026

Contract Type: Contract

Shift Type: 8-hour shifts, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, rotating every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Develop and manage daily patient lists for the Palliative Care team
Review patient charts and assess individual needs
Initiate and facilitate goals of care discussions with patients and families
Educate patients and families on disease processes, advance directives, and POLSTs
Lead family meetings to support complex decision-making
Provide compassionate end-of-life discussions and support
Attend daily interdisciplinary rounds to coordinate care

Qualifications

2-5 years of recent experience in palliative care, hospice, ICU, or oncology settings
Active California RN license required
BLS certification from AHA required
ELNEC certification preferred but not required
Strong acute care and clinical thinking skills
Excellent communication and organizational abilities
A genuine desire to serve patients, families, and team members

Knowledge & Skills

Palliative care principles and end-of-life support
Acute care and clinical assessment skills
Proficiency with Cerner EMR
Ability to facilitate sensitive conversations with empathy
Flexibility to adapt to changing patient needs
Strong time management and prioritization skills
