Hours per Week: 40

Candidate Pay Rate: $70 per hour

Roles & Responsibilities

Provide skilled physical therapy services to patients in their homes
Develop and implement individualized treatment plans
Conduct SOC visits especially for orthopedic patients
Collaborate with interdisciplinary team for coordinated care
Document patient progress in HCHB EMR system

Qualifications

Active Physical Therapist license in California
BLS certification from American Heart Association
Minimum 1-2 years of home health experience preferred
Experience with HCHB (HomeCare HomeBase) EMR is a plus

Knowledge & Skills

Strong knowledge of orthopedic and geriatric conditions
Excellent communication and patient education skills
Ability to work independently and manage daily caseload
Proficient in HCHB or similar home health EMR
