LPN/LVN - Long Term Care

Location: Breckenridge, MN

Start Date: 07-06-2026

End Date: 10-05-2026

Contract Type: Travel Contract

Shift Type: Rotating shifts: 6:00 AM - 2:30 PM, 2:00 PM - 10:30 PM, and 10:00 PM - 6:30 AM NOC; cycle scheduling; every other weekend and holiday required.

Hours per Week: 36

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Provide compassionate nursing care to residents in a long-term care setting
Administer medications and treatments as prescribed
Monitor resident health and report changes to RNs
Collaborate with CNAs and RNs to ensure quality care
Maintain accurate documentation in PointClickCare EMR
Create a home-like environment that promotes resident well-being

Qualifications

Valid LPN license in Minnesota
BLS certification required
Experience in long-term care preferred but not required
Strong teamwork and communication skills
Ability to work rotating shifts including weekends and holidays

Knowledge & Skills

Knowledge of PointClickCare EMR
Understanding of common diagnoses like CHF, COPD, Alzheimer's, Parkinson's, Diabetes
Skills in medication administration and minimal IVs
Compassionate bedside manner
Ability to thrive in a team-oriented environment
