Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide comprehensive physical therapy services to patients in their homes
Conduct initial evaluations and develop individualized care plans
Perform start of care, recertifications, and resumption of care visits
Educate patients and families on therapeutic exercises and safety strategies
Collaborate with interdisciplinary team for coordinated, patient-centered care

Qualifications

Valid North Dakota Physical Therapy License
Current BLS certification
Valid Driver's License and auto insurance
Home Health experience preferred
OASIS training preferred

Knowledge & Skills

Home health and OASIS documentation
Time management and independent work
Compassionate patient communication
Ability to manage a caseload of 5-6 visits per day
