Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts (8:00 AM - 5:00 PM)

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Provide in-home physical therapy services, including evaluations, re-evaluations, and visits
Perform start of care, resumption of care, recertification, and discharge assessments
Develop and implement individualized treatment plans for patients with CHF, ortho conditions, and pneumonia
Collaborate with clinical managers and educators for patient care support
Maintain accurate and timely documentation using provided tablet

Qualifications

Current BLS certification required
Graduate of an accredited Physical Therapy program (new grads welcome, experienced preferred)
Ability to work independently and manage a caseload
Strong communication and documentation skills
Comfortable with home health settings and rotating weekend schedule

Knowledge & Skills

Experience with home health or outpatient therapy
Knowledge of orthopedic, cardiac, and respiratory conditions
Proficiency with electronic health records and mobile technology
Time management and organizational skills
Compassionate patient-centered care approach
