Contract Type: Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM; rotating Saturdays once a month

Hours per Week: 40

Candidate Pay Rate: $60 per hour

Roles & Responsibilities

Assess and evaluate patients' functional status and develop personalized treatment plans
Provide physical therapy services per physician's plan of care in home health setting
Educate patients and families on therapeutic exercises and use of assistive devices
Collaborate with the home health team to ensure comprehensive patient care
Document patient progress and adjust treatment plans to achieve optimal outcomes
Supervise Home Health Aides and participate in case conferences

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab setting
Current BLS certification required
Excellent oral and written communication skills
Strong organizational and time management abilities

Knowledge & Skills

Strong clinical assessment and diagnostic skills
Knowledge of home health regulations, Medicare Conditions of Participation, and Title 22
Proficiency in patient education and family training
Ability to work independently and manage a caseload across multiple territories
